This young woman came to our office hoping to improve her smile. She was born with multiple congenitally missing teeth, including an upper incisor, and presented with a Class III bite and deep underbite. Prior treatment years ago had involved closing spaces and placing a bridge rather than maintaining space for future implants. Unfortunately, this shifted her midline and placed the roots in unfavorable positions, significantly limiting restorative options and ideal tooth proportions.
After careful planning with our orthodontic partner, it became clear that fully correcting her bite would require additional extractions and still would not allow for four naturally proportioned upper front teeth. Before orthodontic treatment began, we discussed that achieving an esthetic result would likely require restoring five to six front teeth using a canine-substitution approach.
At the end of orthodontics, the patient chose a more conservative path and preferred not to treat additional teeth. Working within the limitations of the existing root positions (still not ideal), we used advanced restorative techniques (including pink porcelain), to camouflage discrepancies and create a balanced, natural appearance.
Most importantly, the patient was happy with her result and that is always our ultimate goal.
